Hormonal Questionnaire

This is a questionnaire that must be completed before starting any hormone replacement method. On the day of your visit to our office, we will complete your general, individual history, as well as a specific “hormonal” history that is necessary to select the appropriate replacement method. Additionally, if you are taking any medications (including hormone replacement creams or other medications), it is important to mention them or, more easily, bring the medicine packages with you. It is particularly important to understand your personal history and the medications you are currently taking, as this is the most effective way to collect all the necessary information. Note, however, that you do not need to bring the vitamins or other supplements you may be taking with you during this time. Thank you, and we look forward to meeting you in person!
