This is a questionnaire that must be completed before starting any hormone replacement therapy (HRT). On the day of the visit to our office we will complete your typical, individual history, as well as a specific / special ” hormonal ” history that is necessary to select the appropriate hormone therapy. In addition, if you are taking any medications (including hormone replacement creams or other medications) it is important to mention them or, more easily, bring the medicine packages with you. It is especially important to know what It concerns your personal history and the medications you follow and this is the best way to gather all the necessary information. Note, however, that you do not need to take the vitamins or other supplements you may be taking with you during this time. Thank you and we look forward to meeting you up close!
